Olive Oil and Endometriosis: How Anti-Inflammatory Fats May Help Ease Symptoms

Living with endometriosis means managing chronic inflammation—but your diet can help. Research suggests that extra virgin olive oil’s antioxidants and anti-inflammatory compounds may support symptom management as part of a Mediterranean-style diet.

Millions of women live with endometriosis, a chronic inflammatory condition where tissue similar to the uterine lining (endometrium) grows outside the uterus, including on ovaries, fallopian tubes, or in the pelvic lining. When this misplaced tissue thickens, breaks down, and bleeds with each period cycle, blood gets trapped and can lead to inflammation, cysts, and scar tissue. This can cause women to have painful periods, chronic pelvic pain, heavy bleeding, fatigue, and infertility.

Abundance of micronutrients
Extra virgin olive oil contains antioxidant and polyphenol content such as vitamin E, squalene, and oleocanthal, which boost the anti-inflammatory component of your diet. Oleocanthal is a phenol compound found in extra virgin olive oil that has been studied for its anti-inflammatory characteristics similar to ibuprofen, a popular over-the-counter pain medication used for endometriosis. [3] It is also responsible for the burning sensation that occurs in the back of the throat when consuming a fresh extra virgin olive oil. 

Free radical relief 
With endometriosis, ongoing inflammation can fuel pain and lesion growth, while oxidative stress may worsen tissue damage and sensitivity. The antioxidants in extra virgin olive oil help neutralize free radicals (unstable molecules that can damage the DNA inside cells) and its anti-inflammatory compounds can help modulate inflammatory pathways. This has the potential to ease pelvic inflammation and discomfort. Diets high in saturated fats—commonly found in oils like coconut and palm as well as in animal fats like butter and tallow  —have been associated with increased inflammatory markers, while monounsaturated and polyunsaturated fats are linked to more favorable inflammatory responses. Choosing extra virgin olive oil as a primary fat, instead of saturated fats, can help shift the overall fat profile of the diet toward one that better supports inflammatory balance and long-term health. This simple swap aligns with broader evidence showing that unsaturated fats play a role in lowering inflammation and reducing the risk of chronic disease as part of a heart-healthy, anti-inflammatory eating pattern.

Foundational support for omega-3s
Extra virgin olive oil contains omega-3 fatty acids, which are known for anti-inflammatory properties, making them a valuable component in managing endometriosis symptoms. Some research also shows that omega-3s may help to reduce endometrial tissue itself [1].  These healthy fats can help combat fatigue and improve mood while supporting overall well-being. While olive oil is not a significant source of omega-3s (about 0.7 mg per 100g), the Mediterranean diet emphasizes omega-3-rich foods like fish, as well as highlighting extra virgin olive oil as the primary source of fat to supplement these healthful benefits.
